Process to Appeal an Excluded Drug
Members will need to fill out the RX Appeal Form and send to the address indicated on the form.

To complete the appeal you will need to include a few things:
1.  A detailed letter from the member stating why they would like the drug to be covered.
2.  A letter from the prescribing doctor indicating what other medication was prescribed previously, at what dosage, and what duration.
3.  An explanation from the prescribing doctor as to why the excluded drug is a right fit for the member's condition.  The doctor may want to include articles from peer review journals.

Once the member has all this information and mails it in, it can take up to 30 days for UHC to render a decision.
